After meeting with the primary care physician, patients have the opportunity to spend time with a Health Coach, whose responsibility is to coach and provide patients with the tools they need to better manage their life as well as their disease.

They provide coaching for self-management support that promotes fundamentally focused education that spotlights any specific needs that their disease might require. Physicians Health Choice
A Medicare Advantage Plan Organization that offers health insurance plans in Texas and Florida. Physicians Health Choice contracts with the government agency who runs the Medicare program, the Centers for Medicare and Medicaid Services (CMS).
